The below form, will ask you several questions, confirming that your child is eligible for the program and that you, as a parent or guardian understand the commitment you are undertaking not only for your child, but for yourself.
On the final phase (there are five), you will be asked if you have
read and understand the
2008 Parent Handbook
which includes the Parent Code of Conduct and Volunteering information.
The 2008 Pop Warner Medical Form
is required before taking part in the program. You should review the
information required on that document prior to registering.
If you know there are some areas that you would like to volunteer for, you can enter that data on the final phase, or use the Contact Page to let us know.
You will also want to print out the final page, so you should have a working printer available to you before starting this process.

Each Family will recieve Two (2) 2008 Sports Trip Raffle Tickets with registration and is responsible for selling a Minimum of eight(8) additional tickets. This is a new fundraising effort this year.
We realize there is currently no place to check off Cheer or Football, or Gender. I will get those options online shortly, but if need be, please use the Comments field to provide this information.
