Welcome to 2008 Pre-registration , The Merrimack Cardinals are a Youth Football and Cheer program for ages 5-15.

The below form, will ask you several questions, confirming that your child is eligible for the program and that you, as a parent or guardian understand the commitment you are undertaking not only for your child, but for yourself.

On the final phase (there are five), you will be asked if you have read and understand the 2008 Parent Handbook which includes the Parent Code of Conduct and Volunteering information.

The 2008 Pop Warner Medical Form is required before taking part in the program. You should review the information required on that document prior to registering.

If you know there are some areas that you would like to volunteer for, you can enter that data on the final phase, or use the Contact Page to let us know.

You will also want to print out the final page, so you should have a working printer available to you before starting this process.

Age

• Particpant must be at least five (5) and no older than 15. Weight limits apply for Football.

Documents Required:

• Birth Certificate
• 2008 Pop Warner Medical Form
• Final 2007/2008 Report Card (when available)

Costs:

• $140.00 - Football Per Child (includes all fees and Fundraising Raffle)
• Cheerleaders will be required to have the following items but may use items they purchased from last year:

    $21.00  Bag

  Uniform, sneakers, bloomers and body suit are provided.
• Cleats are not provided and must be purchased separately.

Families with multiple children: $240 for two and $335 for three. If we are lucky enough to have a family with more than three children, please contact Rick (see below).

Each Family will recieve Two (2) 2008 Sports Trip Raffle Tickets with registration and is responsible for selling a Minimum of eight(8) additional tickets. This is a new fundraising effort this year.

We realize there is currently no place to check off Cheer or Football, or Gender. I will get those options online shortly, but if need be, please use the Comments field to provide this information.
